We are also offering onsite optometry exams. The exam will be operated by a professional staff that will provide services to eligible children in a ten county area of southwest Missouri.
Students who qualify will be given an initial exam and treatment will be provided as determined by the examining optometrist. Details of the exam and care
If you are interested in this program for your student, please fill out the information form. Make sure you complete all information which includes a consent agreement and statement that must be filled out and signed by the parent/ guardian. If your child was recently seen by our OCHC Hermitage Optometry Clinic, a new consent form will need to be filled out because of the new school year.
Appointments are limited. Priority is given to students who have the greatest dental need, as well as those who return the completed paperwork promptly.
