About Us!
Ozarks Community Health Center, a proud federally-qualified health center, continues its mission to provide comprehensive health services to the communities of Hickory, Dallas, Polk, Cedar, and Dade counties. The Center offers a full spectrum of care, including Medical, Behavioral Health, Dental,
and Optometry services, complemented by a mobile Dental Bus that serves local schools and communities.
Timeline of Key Developments:
2006: The foundation was laid with the formation of the Ozarks Resource Group by four local health department administrators, aiming to address critical healthcare needs of the community.
2007: Hickory County organizers, through the Missouri Primary Care Association (MPCA), secured a grant for a needs assessment that highlighted a service gap between Springfield and Kansas City serviced by Federally Qualified Health Centers.
2010: The evolution into what is now known as the Ozarks Community Health Center began.
2012: Secured funding with another opportunity for a 330 grant.
2015: Significant strides were made with the establishment of a location in Urbana, Missouri.
2016: Ozarks Community Health Center’s reach was expanded by incorporating the CMH Miles for Smiles Dental Clinic and Mobile Dental Unit in Bolivar.
2018: School-based behavioral health services began.
2019: Dental services extended to Greenfield, Missouri, maintaining its expansion momentum.
2022: The completion of the Urbana Medical Center enhanced healthcare accessibility for Dallas County residents.
2023: Greenfield Dental Clinic celebrated their grand opening.
2024: Stockton Dental Clinic was unveiled. The new Hermitage Dental Clinic building was unveiled and Chiropractic Care was added to OCHC Urbana Medical Center.
2025: Presently, Ozarks Community Health Center is excited to announce the ongoing construction of the Hermitage Medical Center/Complex and the first ever optometry service in Hickory county opened April 1st at the OCHC Hermitage Optometry Clinic.
