Patient Care Approach

At Ozarks Community Health Center, our services are guided by the Patient Centered Medical Home model for healthcare.  PCMH emphasizes the partnership between the patient, provider, and care team. We provide patient centered, evidence-based, coordinated care that is tailored to you and your individual needs.  You are the most important member of your care team!  We will get to know you as a whole person, not just a diagnosis.  We will work together with you and your family to answer your questions and concerns, and help you make the best treatment and health decisions that work for you.  We will assist you in setting personalized health goals and support you in achieving them.

The PCMH care team includes you and your family, your Primary Care Provider, provider’s nurse, Nurse Care Manager, Community Health Worker, Dental Team, and Behavioral Health Consultant.  If you see another health provider or specialist, please let us know so we can coordinate to give you the best possible care.

 

Care Management

Our Nurse Care Managers help patients with chronic and high-risk diseases to make a plan to control and manage your condition.  Just as no two people are alike, no two care plans are alike.  You and your Care Manager will set goals that make sense for your life and your situation.  Care management services are provided at no cost to the patient.  If you have diabetes, high blood pressure, high cholesterol, asthma, COPD, or chronic pain, you may qualify for care management.  Ask your provider for more information.

 

Behavioral Health Consultant

Our Behavioral Health Consultant is a coach for chronic condition management.  The BHC can meet with you for 6 free sessions to provide you with tools to manage many chronic conditions.  This type of service works well with smoking cessation, anxiety and depression management, diabetes, weight loss, and chronic pain.  Our BHC is a licensed clinical social worker with special training in chronic disease management.  Ask your provider for more information.
