Community Health Worker Services

Community Health Workers (CHWs) are public health workers who serve as a liaison between the community and health/social services.  CHWs help patients and their families navigate their health journeys by accessing resources, increasing health education, and helping to adopt healthy behaviors and lifestyles.  CHWs can come to your home or meet in the clinic to discuss your needs and find the resources and services to help.

CHWs can help with:

  • Scheduling appointments for medical, dental, or behavioral health services
  • Food pantry
  • SNAP
  • WIC
  • Transportation
  • Housing assistance
  • Utility assistance
  • Medicaid application assistance

  • Foster transition services
  • Budgeting
  • Job search assistance

CHW Children and Family Initiative

Our Children & Family CHW helps all families, including those in the following circumstances:

  • Children in the care and custody of DSS (foster children/placement)
  • Children or youth in alternative care
  • Children receiving adoption or legal guardianship subsidy
  • Children in relative or kinship care (e.g. grandparents caring for grandchildren)
  • Former foster care youth under the age of 26 who were in foster care on their 18th birthday
  • Youth involved in youth services/juvenile justice

At OCHC, our Community Health Workers (CHW) can help with your insurance needs.
This service is FREE to all of our patients.

We are available to help answer questions about health insurance, explore coverage and compare plans.

Medicare Assistance

Missouri SHIP is the Missouri State Health Insurance Assistance Program that has a highly trained team of volunteers who offer free, unbiased assistance to help individuals looking for help navigating the Medicare program.

Ozark Community Health Center (OCHC) has a partnership with Missouri SHIP which allows you to make an appointment with them by calling 1-800 390-3330 or by clicking the MO SHIP button below.  We encourage everyone to review their Medicare plan information annually, especially during Open Enrollment October 15th – December 7th.
