Transportation Resources

If covered by Medicaid: Medicaid Transportation Management (MTM) can be contacted 24 hrs/day 7 days per week for transportation pertaining to medical need (ie: d/c or Dr’s appointment).

No advance appointment for transport needed if it’s a “discharge” from the hospital, ASC, or  ER. 

If you live in an urban county, call at least two (2) days before your appointment, unless your trip is urgent.

If you live in a basic or rural county, call at least three (3) days before your appointment, unless your trip is urgent. They are not always able to find someone to transport.

It can be difficult to find a wheelchair van, ok to request CMH Health Transit* for wheel chair vans, but no guarantee that they will get the trip.

Nurse, social worker or physician has to be caller to set up discharge. (per MTMs rules).  MTM has 3 hr window to find transport that will accept bid when pt is d/ced from hospital.

  1. Medicaid #
  2. Home address
  3. Dr’s appointment date, time, Dr’s name, address

 

For hospital/ER discharge, staff should provide: 

  1. Medicaid #
  2. Home address (cannot be going to friends)
  3. Home phone #
  4. Room #,  nurse’s station phone # and hospital
  5. Can pt transfer to regular vehicle or require w/c van?                              
  6. Does pt need wheelchair or O2?

Medicaid has door to door service which requires:

  • Letter from PCP on Letterhead w/ Pt MCD # – DOB – Name
  • Reason why they need the door to door service (which allows them to go into home & get pt and take them to the actual office of the provider).This needs faxed to 1-866-269-8875.**It is good for 1 year and the Provider office is responsible for keeping up w/ when it is due again – (but Logisticare tries to give a warning when close to expiration).  If not, the service will be denied once expired. 

Medicare patients:

Some Medicare replacement plans will offer transportation.

Have them look on back of their MCR Advantage card for customer service or transportation.

 

If not covered by Medicaid or a Medicare replacement plan:

*CMH Health Transit  777-5165, Wheelchair van and may accept bids from MTM or private pay.

Monday – Friday 7am til about 4:30pm. Saturday not running unless set up ahead of time (used to run for dialysis on Saturdays but has no pts currently).

 

Bolivar Taxi 326-8294  private pay only. Available 8-5pm Monday-Friday. Til noon on Saturday.

Roadrunner Cab- 417-326-3027 Will travel outside of Bolivar, including long-distances, weekends and holiday hours.

Ability-417-631-8944 or cell 234-7073. Does not have wheelchair lift. Pt must be ambulatory or able to transfer. Has driver in Bolivar in area. Accepts credit card, checks or cash.  Does not accept MCD. Willing to transfer psych pts.

Medi Transit (Now Cox EMS)  417-869-5559, van or wheelchair lift van. Private pay. Pt must be able to transfer out of wheelchair at destination or have arrangements made.  W/C van quits running at 7 p.m.

Bolivar’s Blessings-417-327-0926 Amy Dixon and her husband offer free rides on Fridays for running errands, Dr’s appts, etc.  They may be able to do hospital d/c’s depending on their schedule in Polk County. You can request a ride through Facebook page “Bolivar’s Blessings” or by calling number listed.    www.facebook.com/bolivarblessings. (From Countryside Assembly Church).

New Growth Transit, LLC Benton, Cedar, Dallas, Hickory and St. Clair

  • Grant funded network of transportation volunteers with goal to become official MTM within the next 4 years
  • They currently serve residents in Hickory, Cedar, St. Clair, Benton counties – and anticipate serving Dallas Co residents beginning 1/1/23

They transport wherever these patients need to go – including all locations of CMH facilities

Working to continue growing to cover an even larger service region, so anticipate more counties to be added over time

  • They can transport for any reason (clinical or non-clinical, i.e. grocery shopping, hair appointments) and often pair driver with resident for long-term relationship development
  • To request transportation, anyone may call their single request line, 417-283-7991. Their request line is available Mon-Fri, 7:30a – 5p, but rides can be provided all hours once scheduled (e.g. evenings, weekends).

They recommend requesting with 48-hours’ notice.
