MEDICAL | DENTAL | VISION BENEFITS
HEALTH INSURANCE:
Each full-time employee will receive the Low Health Insurance plan at no cost. Employees may add their dependents to their plan. We also offer a High Health Plan Option to employee and dependents. If an employee chooses the Low Option, OCHC will deposit $75/pay period into the employees’ Health Savings Account (HSA)

DENTAL & VISION INSURANCE:
Each full-time employee will receive the Low Option Dental plan at no cost. Employees may add dependents to this plan. A buy-up option is also available.

Short-term and Long-term Disability:
Full-time employees will be covered under a long-term disability policy, at no cost to the employee. Employees also have the option to enroll in a voluntary short-term disability policy. Premium rates for the short-term policy are based on employee gross earnings and can be calculated at the employee’s request. The long-term disability policy becomes effective after 90-days and lasts for a period of time, depending on the employee’s age.
Life Insurance:
Each full-time employee will receive a $100,000 life insurance policy at no cost to the employee.
Voluntary life insurance policies are also available for employee, spouse, and/or child(ren).
Accident Coverage:
Full-time employees also have the option of purchasing accident and hospital coverage and a cancer care plan through SunLife. More information and rates are available upon request.
401K BENEFITS
Retirement:
All part-time and full-time employees have the option to make contributions to a 401k. If an Employee contributes 5%, OCHC will match 1.5%. If an Employee contributes 10%, OCHC will match 3%. To make any changes to your 401K or sign up, download the form below.

ANNUAL HOLIDAY BENEFITS
Annual Holidays:
Part-time employees will receive 4 hours for each annual holiday and full-time employees will receive 8 hours. The annual holidays are listed below:
• New Year’s Day
• Memorial Day
• Independence Day
• Labor Day
• Thanksgiving Day
• Christmas Eve
• Christamas Day
PAID TIME OFF BENEFITS
Paid Time Off:
All part-time and full-time employees will accrue Paid Time Off on a pro-rata basis, depending on their hours worked each pay period. The total PTO hours an employee will accrue each year are based on the employee’s total years of service. See Below:
Non-exempt employees
Less than 1 Year of Service 72 Hours (9 days)
1-5 Years of Service 144 Hours (18 days)
5-10 Years of Service 184 Hours (23 days)
>10 Years of Service 216 Hours (27 days)
Exempt employees
Less than 1 Year of Service 84 Hours (10.5 days)
1-5 Years of Service 168 Hours (21 days)
5-10 Years of Service 216 Hours (27 days)
>10 Years of Service 248 Hours (31 days)
Employees can accrue the maximum PTO balances below:
Non-exempt employees
0-5 Years of Service 360 Hours (45 days)
5-10 Years of Service 460 Hours (57.5 days)
>10 Years of Service 540 Hours (67.5 days)
Exempt employees
0-5 Years of Service 420 Hours (52.5 days)
5-10 Years of Service 540 Hours (67.5 days)
>10 Years of Service 620 Hours (77.5 days)
Employees may choose to cash out up to 80 PTO hours per year in addition to their normal pay. However, employees must maintain a minimum balance of 120 PTO hours in order to do this.
