OCHC Transportation provides rides to all OCHC appointments in Hermitage and Urbana including:

• Behavioral Health
• Optometry
• Chiropractic

Free to Medicaid patients

Sliding Fee Scale discounted rates: (per 20 miles round trip)

• Level 1 – $1
• Level 2 – $2
• Level 3 – $3
• Level 4 – $4

 Self-pay rates:

• 0-5 miles: $20.42 ambulatory, $26.71 wheelchair
• Each additional mile: $1.27 ambulatory, $1.50 wheelchair
