*$30 nominal fee per visit for preventative/diagnostic services (includes x-rays); $30 fee for exams (includes x-rays); $30 per procedure for fillings and extractions and other services included in the Basic Dental Sliding Fee Scale.
Basic Dental Services include, but not limited to: Cleanings, X-rays, Exams, Fillings and Extractions
Dental – Advanced Services
