We are also offering onsite optometry exams. The exam will be operated by a professional staff that will provide services to eligible children in a ten county area of southwest Missouri.

Make sure you complete all information which includes a consent agreement and statement that must be filled out and signed by the parent/ guardian. If your child was recently seen by our OCHC Hermitage Optometry Clinic, a new consent form will need to be filled out because of the new school year.
