Triage Nurse RN/LPN – FT

Job Responsibilities

• Assists patient to examination room.
• Gathers and documents initial patient subjective data.
• Performs and documents vital signs as directed.
• Assists provider in-patient care as requested.
• Maintains patient exam room in compliance with OCHC Infection Control policies.
• Maintains inventory of supplies needed for duties.
• Receives and handles patients’ medical related telephone calls, relaying information to patients/ others as directed by provider, documenting in the medical record information relayed.
• Assures previous medical information relating to visit is present at time of appointment.
• Responsible for reviewing, correcting, and updating clinical medical information .
• Supports front office, when needed.
• Participates in QI activities as needed.

LAB ASSISTING DUTIES:

• Is responsible for the physical collection of lab tests.
• Is responsible for keeping records in an orderly fashion for lab, filing test results in medical records.
• Is responsible for reporting lab results to the provider per P/P.
• Maintains inventory of supplies needed for duties.
• Follow the Laboratory’s P/P for specimen handling and processing, test analyses, reporting and maintaining records of patient test results.
• Performs and maintains records that demonstrate that proficiency testing samples are tested in the same manner as patient samples.
• Adhere to the Laboratory’s quality control policies, document all quality control activities, instrument and procedural calibrations and maintenance performed.
• Follow the Laboratory’s established corrective action P/P whenever test systems are not within the Laboratory’s established acceptable levels of performance.

SELF EVALUATION

• Identifies problems that may adversely affect test performance or reporting of test results and corrects the problems or immediately notifies OCHC Laboratory personnel.
• Document all corrective actions taken when test systems deviate from the Laboratory’s established performance specifications.
• LPN Case Manager
• Is responsible for prior authorizations on patient prescriptions
• Is responsible for assisting the QI team in managing patients with chronic conditions in their care.
• Is responsible for complex patient nursing calls.
• Assists patients with insurance issues and obtaining services they need.
• Completes Hedis and other insurance quality assurance reports in a timely manner.
• Responsible for completing precertification forms for insurance companies and forwarding to appropriate parties.
